It's not a song about miscarriage, but wishing it happened since the narrator does not feel like having the child.

I must say my interpretation was partly inspired by the high rate of abortion in India. But here the couple went to India for an abortion first, and during the scan, they are very surprised to see tiny twins, so a miscarriage is about to happen.
